How it works
Request your records in 4 steps
We’ll help you understand what to ask for and where to send it. Processing times vary by facility, but we’ll stay with you through each step.
1) Tell us what you need
Share the type of records (lab reports, imaging, discharge summary, prescriptions) and the date range.
2) Verify identity
For privacy, facilities may require an ID copy and a signed authorization form.
3) Submit the request
We’ll guide you on the correct department (MRD/Records) and the best way to submit (email/in-person).
4) Receive and review
When you receive your documents, we can help you check that everything you requested is included.
What to include in your request
Including the right details helps avoid delays and repeat visits.
Add: full name, date of birth, phone number, facility/doctor name (if known), visit dates, record type, delivery preference (email/print), and a signature. If you’re requesting for a family member, include proof of relationship/authorization where required.
